Add a question to a library

 It is not possible to add questions to a question library that is System defined.


Follow these steps to add a question to a library:


  1. Click Add Questions to display a new question panel. 

  2. Type in the Question and use the toolbar to format the text of the question.

  3. To relate this rule to regulatory content, click select a rule.  

  4. Choose an Answer Type. There are many types of answers:

    1. Yes No Partial NA Don't Know

    2. Free Answer — The user can type in an answer.

    3. Yes No NA Don't Know

    4. Multiple Choice — Choose this option and then click +Add an answer to add two or more answers.

    5. True or False — The user can choose either true or false.

    6. Yes No — The user can choose either Yes or No.

    7. Agree Disagree — The user can choose either Agree or Disagree.

    8. Checklist —  Choose this option and then click +Add an answer to create a list of answers that can be checked off.

    9. Yes Yes with CCW No NA Not Tested

    10. Yes No Alternate Response


  5. Check the Conditional Logic box if you want this question to fork to a different path on specific answers. A checkbox will appear that corresponds to each of the answers. Check the box for each of the conditional answers that you want to trigger a question branch.

  6. Optionally, add Reference Text that provides additional information to the user answering the question.
  7. Optionally, you can add one or more Reference Links that can help the user answer the question. Click +Add a link and paste the URL of the webpage that you want to appear.
  8. Click Submit.


The new question will appear on the library page. Click the + Add a Question here link to add another question.
